Your First Dermal Filler Appointment: How to Prepare in San Antonio, TX

Nervous about your first filler visit? Here is exactly what happens before, during, and after, from the consultation deposit to your two-week follow-up.

Start With a Consultation and a $50 Deposit

Every first-time filler patient at Fountain of Youth Élite begins with a conversation, not a syringe. Book "Any Service" under Élite Consultations on our patient platform, in person at our Medical Drive clinic or by video. The 30-minute visit covers what bothers you, your medical history, any past injectables, and what a natural result looks like for your face. A $50 deposit holds the appointment and is credited toward whatever treatment you choose. It is non-refundable for no-shows, and one reschedule is allowed.

If you already know you want dermal filler for nasolabial folds or marionette lines, you can book that 45-minute treatment directly under Élite Injectables as a new or current patient. The consultation happens at the start of that same visit, and we only proceed if we both agree on the plan.

What to Avoid in the Week Before Filler

A little preparation reduces bruising and swelling and makes the appointment smoother. Here is what we ask of patients in the days leading up to treatment.

  • Alcohol: skip it for 24 to 48 hours beforehand; it thins the blood and increases bruising.
  • Blood-thinning supplements and over-the-counter medications: fish oil, vitamin E, ginkgo, aspirin, and ibuprofen for about a week, but only if your prescribing clinician says it is safe. Never stop a prescribed blood thinner on your own.
  • Dental work: schedule cleanings, fillings, and other dental procedures at least two weeks before or after filler, since dental bacteria can travel to fresh filler and raise infection risk.
  • Active skin problems: cold sores, acne flares, or rashes near the treatment area mean we postpone. Tell us if you are prone to cold sores so we can plan ahead.
  • Vaccines and illness: if you are sick or just had a vaccine, let us know; rescheduling is often the better call.
  • Pregnancy or breastfeeding: filler is not performed during either.

On the day, arrive with a clean face and no makeup on the treatment area, eat a normal meal so you are not lightheaded, and bring a list of your medications.

Your Filler Appointment, Step by Step

First visits usually run about 45 minutes. Here is how that time is spent.

Mapping and Photos

Your injector studies your face at rest and in motion, then marks the planned injection points. We take "before" photos from several angles so you can see the change at follow-up. This is also when we confirm the product, the amount, and the per-syringe cost, so there are no surprises.

Numbing

The area is cleansed and a topical numbing cream sits for roughly 15 to 20 minutes. Most hyaluronic acid fillers also contain lidocaine, so comfort improves as the treatment goes on. Patients typically describe pressure and brief pinches rather than real pain.

Injection

Using a fine needle or a blunt cannula, your nurse practitioner places small amounts of filler in layers, checking symmetry and massaging gently as she goes. The injecting itself usually takes 15 to 20 minutes. You are welcome to look in the mirror partway through.

Immediate Results

Volume is visible right away. Expect some swelling, pinpoint marks, and possibly light bruising. We review aftercare, hand you a cold pack, and book your two-week follow-up before you leave.

TimeframeWhat is normal
Day of treatmentSwelling, tenderness, small injection marks, slight asymmetry from swelling
Days 1–3Peak swelling; bruises may appear or darken
Days 4–7Swelling eases; firmness softens as filler settles
Week 2True result visible; follow-up visit to assess

Aftercare for the First Two Weeks

How you treat the area in the first few days has a real effect on your result.

  1. Apply a cool compress gently for a few minutes at a time during the first day to limit swelling.
  2. Avoid pressing, rubbing, or massaging the area unless your injector specifically asked you to.
  3. Sleep on your back with your head slightly elevated the first one or two nights.
  4. Skip strenuous exercise, saunas, hot yoga, and alcohol for 24 hours.
  5. Hold off on makeup over the injection sites for about 24 hours.
  6. Avoid facials, dental work, and other facial treatments for two weeks.
  7. Stay hydrated; hyaluronic acid filler draws in water and looks its best when you are well hydrated.
Your face at day two is not your result. Judge filler at two weeks, in daylight, after the swelling has settled. That is when we look together and decide if anything needs adjusting.

When to Call the Clinic

Normal healing means mild swelling, tenderness, and bruising that improve day by day. Call us at the clinic right away if you notice any of the following, because early treatment matters:

  • Skin that turns white, dusky, blue, or blotchy, especially with increasing pain
  • Pain that is getting worse instead of better after the first day
  • Any change in vision or a severe headache
  • Spreading redness, warmth, or fever, which can signal infection
  • A firm lump that persists past two weeks or appears later

Both of our co-founders trained in emergency medicine, and we would far rather hear from you about something minor than miss something that needed attention.

When Dissolving Filler Is the Right Call

One reassuring fact for first-timers: hyaluronic acid filler is reversible. An enzyme called hyaluronidase breaks it down, with results within 24 to 48 hours. Filler dissolving is used for overfilled areas, filler that has migrated (most often in the lips), asymmetry, lumps, and to clear old product before new filler. It is also part of how we manage the rare vascular complication. We offer a 15-minute virtual dissolving consultation followed by treatment in the clinic, reassess at one to two weeks, and some patients need more than one session. Only HA fillers can be dissolved this way, which is one reason we favor them for first appointments.

Pairing filler with a wrinkle relaxer for expression lines is common, and your injector can map both at the same consultation. Individual results vary, and the goal every time is a result that looks like you on a good day.

First Filler FAQs

What New Filler Patients Ask Before Their Appointment

Does dermal filler hurt?

Most patients describe pressure and brief pinches rather than sharp pain. Topical numbing cream is applied before injection, and most hyaluronic acid fillers contain lidocaine, so the area becomes more comfortable as treatment goes on. Lips and nasolabial folds are usually the most sensitive spots.

How long until I see my final filler results?

You will see volume immediately, but swelling can exaggerate it for several days. Most of the swelling settles within one to two weeks, which is when we schedule your follow-up to assess the true result. Individual results vary, and a small touch-up may be discussed at that visit.

What should I do if I bruise after filler?

Bruising is common and usually fades within a week or two. Use a cool compress gently during the first day, avoid alcohol and strenuous exercise for 24 hours, and skip blood-thinning supplements if your provider has cleared you to do so. Arnica may help; concealer is fine after 24 hours.

Can filler be removed if I do not like it?

Hyaluronic acid filler can be dissolved with hyaluronidase, an enzyme that breaks it down within 24 to 48 hours. We offer a 15-minute virtual filler dissolving consultation followed by in-clinic treatment. More than one session is sometimes needed, and we reassess at one to two weeks.
